Enrollment

We are so glad you're interested in enrolling at South Side Academy! At South Side Academy, you will quickly find that our school will empower your student to be his or her best and that our staff will do whatever it takes for your child to reach their full potential.

Documents required for enrollment are:

  • State-issued identification (parent and/or student)

  • Student's birth certificate

  • Proof of address (i.e. current utility bill, current lease, bank statement, etc.)

  • Proof of income (students 18 and over)

  • Current vaccination record

  • Student's official transcripts

  • Withdrawal letter from student's most recent school previously attended

  • Proof of guardianship, if applicable.


If you would like to complete a paper application, please click the download button and return it to the school. Applications can be received at the school M-F 8:00am - 3:30pm. If you should need further assistance during this process, please contact us at (330) 774-5562.
